Thank you for your interest in our practice. Patients new to our practice requesting an appointment for a consultation can call our office, for Phoenix-Metro (480) 470-4000, Tucson (520) 485-4000, and for Flagstaff (928) 863-1000. Alternatively, you can e-mail us at scheduling@azliver.com supplying the required information and we will call you to arrange an appointment.

Working With Your Insurance

At Arizona Liver Health, we work with patients and insurance carriers to make sure that you receive the care you need. We participate with most major insurance plans. We also accept private-pay (or self-pay) patients.

What to Bring to Your Appointment

We’ve put together quick lists of documents to bring to your Arizona Liver Health appointments. This information usually depends on the type of appointment and whether you’ve been to Arizona Liver Health before.

Initial Consultation

If it’s your first time at AZ Liver and you’re visiting for a screening or consultation, please bring:

  • Photo ID
  • Proof of address, if different from your photo ID address.
  • Your medical insurance card or other applicable information (optional)
  • New Patient Information Packet:
  • Patient Information Forms
  • Privacy/HIPAA Acknowledgement Form
  • Pharmacy contact information
Treatments and Follow-Up Visits

If you’re coming in for a follow-up visit, please bring:

  • Photo ID
  • Your medical insurance card or other applicable information
